In addition to the services listed above, our knowledgeable team has  expertise in providing personal care for seniors that are experiencing  memory loss and confusion due to Alzheimer’s disease or other dementia. 

Skilled Nursing Care

Our experienced nurses  perform in-home health assessments and a wide range of services that  meet either short-term or ongoing medical needs. These include, but are  not limited to:

  • Acute or chronic disease management
  • Blood draws and injections
  • Blood sugar testing
  • Home infusion therapy 
  • Medication setup, administration and oversight
  • Monitor and assess vital signs

Short-Term Transitional Home Care Services

COPD (Chronic Obstructive Pulmonary Disease): We’ll coach proper inhaler use, provide medication reminders and support habits to improve wellness.

  • Diabetes: We will conduct foot inspections and perform blood glucose level checks, as well as provide family education.
  • Heart Attack/AMI (Acute Myocardial Infarction): We’ll provide education and support to develop new lifestyle habits that promote heart health.
  • Heart Failure: We’ll help manage medication adherence, diet and exercise.
  • Pneumonia: We’ll provide RN-led medication management, help manage oxygen therapy and offer personal care.
  • Post-Operative: We will assist with surgical recovery in the comfort of home.
  • Stroke: We will help with activities of daily living (ADLs) like eating,  dressing and mobility, as well as promote fall safety, offer medication  reminders and provide transportation to physician appointments or  therapy (physical, occupational and/or speech).
  • Therapy for Post-Op or Stroke: We also have physical, occupational and speech therapists who can provide rehabilitation in your home.

The First Signs

Alzheimer’s disease is a  neurological disorder that often begins slowly with mild symptoms that  gradually advance. Although some memory loss or changes in function are  normal as we age, if symptoms move quickly or become more frequent, it  could potentially indicate the onset of Alzheimer’s or another form of  dementia.

 Here are some things to watch out for:

  • The inability to remember words, details, directions, people and events
  • Difficulty performing simple tasks (such as nutrition, personal hygiene or grooming)
  • Depression or lack of motivation
  • Forgetfulness and confusion
  • Uncharacteristic irritability
  • Poor decision making or judgment

It’s  also important to note that those who have Alzheimer’s may be at a  greater risk of falling or accidentally injuring themselves.

Licensed and experienced therapists through Quality Home Care will create a customized plan  of care to address your unique in-home therapy needs. We provide these  services for adults and children.

A brief overview of therapy types is provided below. Please contact us for a detailed list of services offered in San Francisco and Marin County or to discuss your specific needs.

Physical Therapy

Also  referred to as PT, physical therapy is a branch of rehabilitative  medicine that uses specially designed exercises and equipment to help  individuals retain or improve their physical abilities.

This  type of service can range from assisting young children with balance  and coordination to hip surgery rehabilitation or stroke recovery.

Speech Therapy

Although  it’s often believed that speech therapy is just about the physical act  of speaking and articulating words correctly, it actually covers a wide  spectrum of services.

This service includes addressing  tongue thrust for infants, swallow therapy for patients of all ages,  cognitive rehabilitation for individuals who have had a stroke or  experienced a traumatic brain injury (TBI) like a concussion and many  other needs. Speech-language pathologists (SLPs) or SLP assistants can  provide this in-home care.

Occupational Therapy

Even  though this type of therapy has the word “occupational” in its  description, it’s not limited to functions needed for employment. While  physical therapy is designed to improve or restore motion and strength,  occupational therapy (OT) focuses on activities that clients enjoy and  need to do in their daily lives — whether that’s as a student, parent,  worker or volunteer.

OT considers the complex  relationship between the client, the activity and the environment in  which the task takes place. This may include household chores, riding  the bus to school and navigating the hallways, changes in routines to  improve safety, driving and nearly any other day-to-day activity.

Provided by  Registered Nurses (RNs), Licensed Practical Nurses (LPNs) or Licensed  Vocational Nurses (LVNs), this in-home service category includes medical  care such as catheter maintenance, wound care, medication management,  blood draws and training for complex equipment such as ventilators.  Nurses are able to meet many advanced care needs in order for your loved  one to be able to remain in their home when receiving these services  and treatments:

  • Medication management and administration
  • Wound care
  • Blood draws and injections
  • Central and peripheral intravenous drug therapy, such as chemotherapy
  • Assistance with medical equipment including ventilators or oxygen tanks
  • Administration and maintenance of catheters
  • PICC line dressing changes
  • Total Parenteral Nutrition (TPN), G-tube or other feedings
  • Epidural mediports
  • Physical, occupational or speech therapy
  • Rehabilitation
